Disc comes in an unusual fold out cover - a 62cmX31cm sheet of thin cardboard folded in the middle, with one side printed and with embossed statuette photograph (Golden Apollo statuette, a prize won) on front cover, inner spread has got a flap at the bottom edge fixed to the right made to preserve the disc and a ca. 305mmX305mm four page colour booklet out of the same thin cardboard as cover (ca. 61cmX305mm, folded in the middle) with liner notes (dated on the 15th of May 1976), photographs and information about songs and awards.
Catalogue number with hyphen - LSY-68004, appears on centre labels, the one without hyphen, LSY 68004, on spine and on inner spread of booklet.
